Without the document we can not eliminate the possibility of scan error.
Otherwise, if the others certificates are represented correctly, please try to scan the document again (pdf and jpeg format).

Have you tried to open it directly from Acrobat Reader?
How is the certificate shows?
Have you tried with a JPEG format?
